Car Rider Name Tag Request Form

If you are planning for your child to be an afternoon car rider during the 2024-2025 school year, we ask that you complete the following form to request a personalized car tag.  Each family will receive TWO copies of the car tag(s) requested.  Any name tags requested prior to Open House will be available for pick up at that event.  Name tags requested at a later date can be picked up at the East Albemarle Main Office, or will be sent home with students on the first day of school (August 8th, 2024).  Car name tags can be requested at any time during the year using this form and will be sent home within two school days of the request being placed.  
To expedite the dismissal process, all parents/guardians should prominently display the car tags on the passenger side of the vehicle (windshield or window) for East staff members to easily read.
